Diabetes Peer Navigator
Virtual · Flexible hours · $50 per 45-minute session
We're hiring a Diabetes Peer Navigator to walk alongside OneVillage members living with prediabetes and diabetes, $50 per 45-minute virtual session. We bring you the members. You bring the lived experience and the training.
About OneVillage
Healthcare is broken. Not because people don't care, but because navigating it alone is nearly impossible. At OneVillage, we're fixing that.
OneVillage is a whole-health employee benefits platform that connects employees with expert navigators, clinicians and specialists across insurance navigation, medical navigation, mental health, nutrition, physical and occupational therapy, chronic condition care, financial wellness, and more. We're not a chatbot. We're not a directory. We're real humans, deeply trained and deeply empathetic, who show up for employees at the moments that matter most.
Think about what it means to have a brilliant friend who happens to know everything about healthcare, benefits, and wellness, someone who picks up the phone, figures it out with you, and doesn't let go until the problem is solved. That's OneVillage.
Our members feel it. The reviews and testimonials we receive daily are humbling: employees telling us we saved their marriage, their finances, their sanity, their lives. We've coordinated emergency medevacs. We've helped members navigate cancer diagnoses, mental health crises, and impossible insurance battles. We've been there for the moments that no one else shows up for.
And we're just getting started.
The role
The result for our specialists: you get to spend your time actually supporting members. We bring the clients, handle the insurance and employer-benefits side, schedule the sessions, pay you reliably, and surround you with 25+ other hand-selected clinicians and specialists you can collaborate with and refer to. No cold outreach. No marketing grind. No insurance-credentialing nightmare. Just the work you trained to do.
Who we're looking for
- Someone who has lived with diabetes or prediabetes, or cared for someone who has, and has turned that experience into a trained practice of helping others.
- Practical and encouraging about the daily work of diabetes: glucose checks, medication routines, food, movement, sleep, stress and the mental load of a chronic condition.
- Clear about the line between peer support and clinical care, and quick to bring in a dietitian, nurse or physician when a member needs one.
- Someone who believes care works best in community. Our diabetes members also work with our registered dietitians, physical therapists and mental health clinicians, and you are the person who keeps the whole plan moving.
What you'll do
- Deliver 1:1 virtual peer navigation sessions (45 min) to members on our diabetes and healthy-weight pathways
- Help members understand their care plan, build daily routines, prepare for doctor visits and stay on track between appointments
- Run the structured check-ins built into our eight-week Whole Health Pathways, including questionnaires at weeks 0, 4 and 8
- Work under the direction of a OneVillage physician or nurse practitioner, who oversees each member's navigation plan, and connect members to our registered dietitians, physical therapists, mental health clinicians and insurance navigators when a need appears
- Support members using GLP-1 medications with the practical side of the routine, in partnership with their prescribing clinician
- Document session notes in our secure portal
What you bring
- Lived experience with diabetes or prediabetes, personally or as a caregiver, and the ability to talk about it with warmth and boundaries
- Community Health Worker (CHW) certification from the state where you live, or the ability to complete it in your first six months. Maryland and Virginia both run state programs, and this is what lets your time be billed to health plans as part of a care team
- Completion of a CDC-approved Diabetes Prevention Program lifestyle coach training (about 12 hours), or the ability to complete it before your first cohort. OneVillage pays for the course
- A National Provider Identifier (NPI), or willingness to apply for one when you start. It is free and we walk you through it
- 2+ years supporting people with chronic conditions in a health, community or employer setting
- Comfort with virtual sessions over HIPAA-compliant video and with simple health-tracking tools
- Reliable internet and a quiet, private home-office setup
Nice to have
- ADCES Diabetes Paraprofessional training, or Certified Peer Specialist credential
- Experience delivering a CDC-recognized Diabetes Prevention Program or an accredited diabetes self-management education program
- Experience with GLP-1 weight-management programs or cardiac risk reduction
- Experience working in a care team alongside dietitians, nurses and physicians
- Fluency in Spanish, Mandarin, or another second language
- Prior work inside an insurance-covered or employer-benefits care model
What we offer
- Clients come to you. Members find us through insurance, employer benefits, and our partner organizations. You never have to do lead-gen.
- We handle the plumbing. Credentialing, scheduling, HIPAA-compliant video, session notes, billing, and insurance eligibility checks are all handled by OneVillage.
- Flexible and virtual. Set your own availability. Add as many or as few client slots as you want. Work from anywhere with reliable internet.
- Real colleagues, not a platform. You'll be part of a tight-knit team of about 25 clinicians and specialists who meet, refer clients across specialties, and genuinely care about each other's practices.
- Mission-aligned pay. Session-based compensation with transparent rates. No volume quotas. No pressure to upsell anything.
Compensation: $50 per 45-minute session
